PHARMACOKINETICS and PHARMACODYNAMICS: AMOXIL ® 12 h is a semi-synthetic group of beta-lactams thiacetazone. It has a broad spectrum of antibacterial activity against many Gram-positive and Gram-negative microbes, and acts by inhibiting the synthesis of cell wall mucopéptida. It is a rapid bactericidal and possesses the safety profile of Penicillins.

AMOXIL ® 12 h is well absorbed; the usual administration of three times a day produces high levels in serum, the presence of food in the gastrointestinal tract does not diminish the absorption significantly, so its use for oral administration is preferred. AMOXIL ® 12 h has an excellent penetration in bronchial secretions and maintains high urinary concentrations.

Approximately 18% of the circulating amoxicillin is bound to plasma proteins. Amoxicillin is rapidly distributed in most tissues of the body and fluids. High concentrations are found in bile and urine. Fails to brain or spinal fluid unless the meninges are inflamed.

Amoxicillin elimination half life is of
1 hour. Major route of elimination is renal tubule; between 60 and 70% of the dose was recovered in urine unchanged, during the first 6 hours after administration of a standard dose; AMOXIL ® 12 h is also partly excretado in urine as an acid inactive peniciloico in equivalent of 10% to 25% of the initial dosage amounts.

Contraindications: Hypersensitivity to Penicillins or cephalosporins.

It may be possible that there is a sensitivity crossed with other lactam antibiotics, for example, cephalosporins.

General precautions: Before prescribe AMOXIL ® 12 h, rule out a possible hypersensitivity to Penicillins and other beta-lactams. Reported serious hypersensitivity reactions, occasionally fatal (anaphylactoid) in patients treated with penicillin. These reactions are more common in patients with history of hypersensitivity to beta-lactam antibiotics.

The presence of rash has partnered Erythematosus (morbiliforme) with glandular fever in patients who received amoxicillin.

Prolonged use can occasionally promote the development of non-susceptible organisms.

The dose should be adjusted in patients with kidney damage.

AMOXIL ® 12 h contains sodium benzoate.

DRUG interactions and other GENRE: the probenecid reduces renal tubular secretion of amoxicillin, resulting in levels high and most protracted of amoxicillin in blood.

Amoxicillin may reduce the effectiveness of oral contraceptives.

Concomitant use with allopurinol may increase the likelihood of skin allergic reactions.

When prescribing anticoagulants concomitantly with amoxicillin, should be an appropriate follow-up, as it has been reported prolongation of prothrombin time.

It is recommended that when performing tests to detect the presence of glucose in urine during treatment with amoxicillin, is used to detect the method of the enzyme glucose oxidase, because high concentrations of amoxicillin in the urine can give false positive results.

Cimetidine, not ranitidine, may increase the absorption of amoxicillin.

MANIFESTATIONS AND MANAGEMENT OF OVERDOSE OR ACCIDENTAL INTAKE

Even if amoxicillin is practically nontoxic and well tolerated in cases of overdose can occur nausea, vomiting and diarrhea which must be treated symptomatic, with special attention to hydration and electrolytes balance. Cristaluria has been observed by amoxicillin, which in some cases leads to kidney failure (see precautions, General).
